That's the surgeon's fault. When she had her tummy tuc, they should have relocated her navel to stretch out the skin of the upper abdomen. Now that she's healed and the skin has scarred down, it would be very difficult to relocate and stretch out the skin, without making many puckers in the lower abdomen.

So, it would be very hard to have it fixed. Of course, it she puts on some weight, the sag won't be as obvious.
